Piperacillin Na 4 g, tazobactam Na 500 mg

Indications/Uses

Lower resp tract, complicated & uncomplicated UTI, intra-abdominal, skin & skin structure infections; bacterial septicemia. Polymicrobic infections. In combination w/ aminoglycoside for bacterial infections in neutropenic adults or childn.

Dosage/Direction for Use

Slow IV infusion/inj Adult & childn ≥12 yr 2.25-4.5 g every 6 or 8 hr. Bacterial infection in neutropenic adult & childn >50 kg 4.5 g every 6 hr in combination w/ aminoglycoside, childn <50 kg 90 mg/kg every 6 hr in combination w/ aminoglycoside. Max: Not to exceed 4.5 g/dose over 30 min. Adult & childn ≥12 yr CrCl 20-40 mL/min Max: 4.5 g every 8 hr, <20 mL/min Max: 4.5 g every 12 hr. Childn <12 yr CrCl ≤50 mL/min 78.75 mg/kg every 8 hr.

Special Precautions

Discontinue use if allergic reactions or bleeding manifestations occur. Antibiotic-induced pseudomembranous colitis. Haemophagocytic lymphohistiocytosis. Overgrowth of non-susceptible organisms. Closely monitor & discontinue use if lesions progress. Perform periodic assessment of haematopoietic function. Periodic electrolyte determination in patients w/ low K reserves or taking medications lowering K levels. Prolonged & high-dose use. Concomitant use w/ vancomycin. Hemodialysis patients. Renal impairment. Pregnancy & lactation. Childn <2 yr. Elderly >65 yr.

Adverse Reactions

Diarrhoea. Candida infection; thrombocytopenia, anaemia; insomnia; headache; abdominal pain, vomiting, constipation, nausea, dyspepsia; rash, pruritus; pyrexia, inj site reaction; increased ALT & AST, blood creatinine, alkaline phosphatase & urea, decreased total protein & blood albumin, +ve direct Coombs' test, prolonged aPTT.

Drug Interactions

Prolonged neuromuscular blockade of vecuronium. Perform appropriate coagulation tests w/ concomitant heparin, oral anticoagulants & other drugs affecting blood coagulation. Reduced MTX excretion. Longered t_½ & lowered renal clearance by probenecid. Increased risk of nephrotoxicity w/ vancomycin. False +ve glucose urine test.
